To Respect, Dignity, and Justice

You have the right to compassionate, confidential, and respectful virtual telehealth care. You will be treated with dignity and remain free from neglect, exploitation, abuse, harassment, racism, or discrimination. We provide inclusive care to all, regardless of race, age, creed, ethnicity, national origin, citizenship, sexual orientation, gender identity, or disability.

To a Safe Digital Environment

You have the right to receive care in a secure virtual setting. We utilize encrypted technologies to ensure your consultations remain private and free from unauthorized intrusion or harassment.

To Access Care Responsive to Your Needs

You have the right to access medically necessary services and accommodations. We aim to align our virtual treatments with your personal life goals. If you have a disability, you have the right to involve support persons in your virtual visits to assist with communication or care decisions.

To Participate in Your Care Decisions

You know yourself best. You have the right to ask questions and make informed decisions about your treatment. We will honor your personal, cultural, and spiritual values. You may also request the participation of a trusted decision-maker, specialist, or interpreter (provided at no cost) during your virtual appointments.

To Have Your Wishes Honored

We respect your treatment decisions. If you are unable to speak for yourself, we will honor the choices made by your designated power of attorney or health care agent. We encourage you to share your advance directives with us so we can integrate your preferences into your virtual care plan.

To Informed Consent and Declination

Your provider will explain your diagnosis, treatment, and prognosis in plain language. You have the right to understand the risks and benefits of any proposed care and the risks of declining it. You may refuse treatment or end a virtual session at any time.

To Privacy, Confidentiality, and Medical Records

You have the right to confidential treatment of all communications. You may access your medical records and clinical notes in real-time through our patient portal. We will provide a separate Notice of Privacy Practices detailing how your digital data is protected.

To Transparent Finances and Feedback

You have the right to request cost estimates and receive clear, itemized billing. If you are experiencing financial hardship, we are committed to finding payment options or assistance regardless of insurance coverage. Additionally, you have the right to voice complaints about your care without it affecting the quality of your future treatment.

As a Patient, You Have Responsibilities

PNW Virtual Health is a digital space of healing. To provide the best care possible, we ask that you:

  • Be Respectful: Treat our virtual providers and staff with the same dignity you expect. Harassment or discriminatory language will not be tolerated and may result in the termination of the provider-patient relationship.
  • Be Accurate: Provide a complete and truthful medical history, including current medications and unexpected changes in your condition.
  • Be Engaged: Inform your provider of your health priorities, ask questions when you don’t understand, and follow the agreed-upon treatment plan.
  • Be Prepared: Ensure you are in a quiet, private location for your virtual visits and notify us promptly if you need to cancel or reschedule.
  • Share Documentation: Provide us with copies of your advance directives or the contact information for your trusted decision-maker.

If you feel your rights have been violated or have concerns about your care, you can file a grievance directly through our administrative office.
